SLS™ Collaborative F2F Events 2018

December 9-12 

We are pleased to announce a series of SLS™ Collaborative activities to be held in conjunction with IHI Annual Forum at the Orlando World Center Marriott in Orlando, Florida. This year we will be featuring several opportunities for networking and learning with fellow collaborative members. Below is the full list of events held by HBHS for your reference. IHI Pre-Conference Learning Lab

Room: Conference Center, Level 1, Crystal Ballroom, Salon J 

Stop Mortality Reviews and Start Learning: Using a real-time audience response system, participants will identify opportunities for improvement in care delivered to simulated patient journeys; quantify barriers to implementing a reliable learning system; and prioritize opportunities for improvement. Participants will gain actionable insights to apply in their organizations. 

SLS™ Leaders will conduct this IHI Pre-Conference Learning Lab; if you have new reviewers this may be a training option but does require registration through IHI.

Tell Your Story

We want to hear your stories no matter how far along you are in the SLS™ journey. We will have resources onsite if you would like assistance in capturing your SLS™ journey either written or in video format. Annual F2F SLS™ Collaborative Dinner

Join us with your breakfast for discussions around key topics and have access to the knowledge of Subject Matter Experts across the SLS™ Collaborative.

Tell Your Story

Share your SLS™ journey with other collaborative members by a storyboard and socialize your story within your hospital through a video interview. 

SLS™ Journey Storyboard

This year we are offering the opportunity for collaborative members to share their SLS™ journey with the visual aid of a storyboard. All we need from you is your story. Your poster should include a title and list of authors (with affiliations and emails). One poster will be made available to each collaborative member hospital, however, several authors may contribute to a single poster. We will print the posters for you and distribute them at the event. Storyboard presentations will occur during the Annual Dinner on December 10. Please use this template to complete your storyboard. 

If your poster is a representation of a research study, you will want to include:

  • Introduction or objective 
  • Methods 
  • Results 
  • Conclusions and/or discussions 
  • Acknowledgements 
 If your poster is a representation of an event or other kind of research project, you may want to forego formal abstract sections in favor of the 5 Ws : 
 
  • Who (introduce the author, organization, or community)
  • What (what did you do? How did you do it?)
  • Where (where did you do it?)
  • When (when did it take place?)
  • Why (what are the outcomes, implications, or future possibilities?) 
  • Acknowledgements

Video Interview

The video is an example of a collaborative member interview from the 2017 IHI Forum featuring Valerie Craig, RN and Vicki Nolen, LVN, MAPD, LSSYB who jumpstarted SLS for the Methodist Health System, TX. This video is condensed raw footage to help prepare you for the interview. The final video will be edited with music and intros added. 

If you wish to have an interview partner we can interview two people at once but you must represent the same hospital. This video material may be used as socialization to help encourage engagement within your hospital.

Regardless of numbers, those interviewed will be featured individually on the website on a dedicated page. Interviews will also be combined for testimonials throughout the website. Make sure to check your institutional guidelines in seeking permissions to socialize externally. 

Interviews will be conducted on December 10 before the Annual Dinner starting at 6:00 PM. Please fill out the contact form below and our Public Relations Specialist, Shelby Hart, will get back to you as soon as possible. Videos will be made available sometime late December or early January 2019 and may be viewed on the website and our YouTube channel.
